Meet Harlee Nason
LCSW, C-ASWCM, CDP
I am a licensed clinical social worker with more than 25 years of experience serving older adults and their families across Massachusetts. I specialize in elder advocacy, complex care coordination, and family support during times of transition, crisis, and long-term planning.
I earned my Master of Social Work from Boston University, along with a post-graduate certification in Marital and Family Counseling. I am a Certified Advanced Social Work Case Manager (C-ASWCM) and a Certified Dementia Practitioner (CDP), with extensive clinical and supervisory experience across healthcare, community-based, and protective service settings.
Throughout my career, I have worked in hospitals, behavioral health programs, adult day health services, and elder service agencies, including senior leadership roles within Protective Services and Elder At Risk programs. I have provided direct care, clinical supervision, program development, and regional consultation, with a strong focus on elder safety, abuse prevention, capacity assessment, and systems navigation.
Most recently, I have served as an Aging Life Care Manager, conducting comprehensive psychosocial assessments, developing individualized care plans, coordinating medical and in-home services, and advocating for clients’ needs across complex healthcare and social service systems. I am deeply committed to promoting dignity, safety, independence, and quality of life for older adults, while offering education, guidance, and emotional support to families and caregivers.
I am a member of the National Association of Social Workers and an Advanced Professional Member of the Aging Life Care Association. My practice is grounded in compassion, clinical expertise, and a practical, solutions-oriented approach tailored to each client’s unique situation.
